Posture in thoracic surgery.

چکیده

It is important in thoracic surgery to find a position upon the operating table which will combine good surgical access with the minimum interference with respiration. Present practice favours a postero-lateral approach for most thoracotomies, and for this the patient is placed with the gobod side of the chest under. The lateral position is maintained by a pad against the front of the chest and by pelvic and buttock supports which prevent rolling or slipping if the table is tilted. A padded wedge or pillow is sometimes placed under the thorax so as to bow the chest, opening the rib spaces on the upper side and so facilitating the exposure. Sellors' " chest-rest" combines the wedge and the anterior support in one piece. This position gives an excellent exposure for the surgeon but interferes with respiratory exchange. It has been stated, as a result of the investigations of Bjorkman and Jacobeus in bronchospirometry (Bjorkman, 1934), that with a patient lying in the lateral position the lower lung has the greater respiratory exchange. This appears to be a fortuitous advantage accruing from a position which has been adopted because of its surgical convenience; but when the chest is open the tidal air into the lower lung is reduced; and, unless methods are adopted to assist ventilation, anoxia develops.

The Relationship of Forward Head Posture and Rounded Shoulders with Neck Pain in Iranian Office Workers

Background: Office workers spend a long period of time behind a computer during working hours. The relation between the posture of sitting during work with computer and neck pain is still debatable. Even though some researchers claim a significant difference in head posture between patients with neck pain and pain-free participants, the FHP (forward head posture) has not always been associate...

The Effect of Spine Strengthening Exercises and Posture Training on Functional Thoracic Hyper Kyphosis in Young Individuals

Background & Objective:  Hyper- kyphosis negatively affects health status, physical mobility, and quality of life. Corrective exercise interventions are often utilized to manage subjects with thoracic hyper-hypnosis. The purpose of this study was to investigate the effectiveness of multimodal spine strengthening exercises (spinal strengthening, spinal alignment and other such exercises for incr...

Influence of scoliosis corrrection in the thoracic spine on posture compensation in patients with King's type II scoliosis (Lenke's IB, IC).

Background. The aim of our study was to assess the course and outcome in scoliosis without spondylodesis in the lumbar spine after surgical correction performed in the thoracic segment with the C-D method in patients with King's type II scoliosis (Lenke's IB, IC). Material and methods. 42 patients with idiopathic scoliosis were operated in the thoracic spine. The Cobb's angle averaged 635 +/- 9...

Video-assisted thoracic surgery for superior posterior mediastinal neurogenic tumour in the supine position

Video-assisted thoracic surgery (VATS) for a superior posterior mediastinal lesion is routinely done in the lateral decubitus position similar to a standard thoracotomy using a double-lumen endotracheal tube for one-lung ventilation. This is an area above the level of the pericardium, with the superior thoracic opening as its superior limit and its inferior limit at the plane from the sternal a...

Coronary artery bypass grafting in cerebral palsy with severe contractures of extremities.

We report on a 76-year-old man suffering from cerebral palsy who underwent coronary artery bypass grafting. Limitation of available bypass grafts and the approach of intra-aortic balloon pump insertion are of great concern because of his specific posture resulting from multiple articular contractures. We describe our planning to survive coronary surgery in this specific situation.

Changes in Blood Gases and Hemodynamic Parameters in Patients Undergoing Lung Resection Surgery and Its Clinical Implications

Introduction: Lung resection surgery is a challenge to thoracic surgeons. Outcome of surgery depends on patients’ tolerance to reduced lung volume and hemodynamic alterations. The present study aimed to investigate the changes in blood gases and hemodynamic parameters in patients undergoing lung resection surgery and the associated clinical implications. Materials and methods: This study includ...
